I have been blessed be connected with equines during my entire life. However, back in 2013, my pony Cupcake, was diagnosed with EMS/IR, then eventually with PPID.  In 2013, her insulin was <200, and was unable to receive guidance how to navigate this via the veterinary clinic.  I was frustrated, she was sore.  This pony was on a dry lot.  I ended up heading to the internet and found the ECIR group.  I posted my test results, and started my case study and never looked back.  They promptly responded that she was severely IR, and that I needed to start the "emergency" diet; which, I did immediately.
